About The ACCESS Questionnaire and Brag Sheet
The Access Questionnaire is a survey used to assess the academic, college/career and personal/social requirements of every student. Students should complete the questionnaire in the beginning of the year so that the school counseling program can address their needs as a result.
The Brag Sheet is designed to provide the school counseling program with detailed information of every student. The Brag Sheet highlights each student’s special qualities, interests and accomplishments.
